Physician House Calls, In-Home Primary Care, Transitional Care Management & Geriatric Medicine

BL House Calls provides physician house calls to patients who are homebound or live too far from their primary care physician. BL House Calls manages medications, wound care, acute illness, referrals, in-home diagnostics, therapy coordination, nursing help, and durable medical equipment needs. Services help patients receive care at home and stay out of the hospital whenever possible.

Our mission

BL House Calls exists to bring quality medical care into the home by helping patients access primary care, transitional care, geriatric support, diagnostic services, referrals, and care coordination when travel to a clinic is difficult.
